We are pleased that you have chosen our practice, and we will do our best to help you with your endocrine needs.

We ask that you download and complete the following forms and bring them with you at the time of your visit.

We also ask that you bring the following items with you at the time of your visit:

  •   Your insurance cards
  •   Any medications you are currently taking
  •   Log book of home glucose readings and your glucose meter, if you have diabetes.
  •   Your co-payment and / or method of payment for services.
  •   A copy of your last labs or test reports (or have your physician fax your records to us ahead of time)

If you do not have insurance, your payment will be expected at the time of your visit. Fees vary depending on the complexity of your problem. Payment may be made by check, cash, visa, master card or discover card.

Welcome!  Thank you for your interest in attending our diabetes education workshops. If you cannot attend a scheduled workshop, please let us know as soon as possible.

Our monthly comprehensive program consists of four sessions:

It is important that you attend all sessions to obtain maximum benefit. (You will be notified ahead of time should there be a change in your class date.)

Comprehensive Program

  • 1:1 Assessment   (1/2 - 1 hr )                                 
  • Diabetes basics class (4 hrs)     
  • Nutrition (3 hrs)                            
  • Diabetes Follow - up (2 hrs)  

We also offer the following workshops:     

  • Annual Diabetes Review  (2 hrs) (held every other month)          
  • Insulin Basics (3 hrs) ( held quarterly)                 
  • Pump Workshops (held based on need)

Please bring these items with you ON THE DAY OF your first session:

  • Your insurance card(s) and a picture ID if not already in our file
  • All completed  forms, your meter and test strips
  • Cash, check or credit card payment for your co-pay or any out of pocket fees. We will bill your insurance if service is covered.
  • Any medications that you will need to take.

You may bring a guest who does not have diabetes. All persons with diabetes must register to take the Living Smart Diabetes program.

Please wear comfortable clothing and come prepared to learn a lot and have some fun.  Free glucose meters are available. All attendees receive free educational materials.
